To many lines on account

I have two lines from Walmart family mobile and when i went to pay my bill online it says i have three lines. When i bought the second phone to activate the second line the activation didn't not work said that the sim card was not valid.

Therefore i went back to Walmart to solve the problem but the lady only gave me a refund and said that it will be find now i have three lines and not even using the third line so now im writing to you so you can fix the problem and take the the third line off my bill.

Lastly my bill is now 113 dollars and that is way to much for only two line so i want you to fix the whole situation and that the 30 extra dollars that im getting charged for. Hope to here from you and have the problem fixed as soon as possiblel. Thanks for the help.
