Billed $54 after cancelling and sent to collections

I decided to switch to Dish Network after being with DTV for about 8 years. I paid my bill to DTV, then called and shut it off 3 days later. The rep I talked to on the phone said I would be issued a credit, but then I was sent a bill the next month for the service. When I called them, they claimed that I still owed them $54.00. I sent them the documentation showing where I had paid the bill, but they turned me over to a collection agency anyway.

I contacted the Better Business Bureau a long time ago but have heard nothing from them yet, and I am still receiving threatening letters every month from the collection agency they hired against me.
