Billed me for phone calls i did not make, threatening to send to collections for $12, ripoff!

They keep sending me a statement for phone calls i supposedly made using their service. But they will not or can not send me any type of original invoice showing where the calls were made to, the dates, times, and telephone numbers i supposedly called!

The bill is less than $12 and now they're threatening to send it to collections! For $12??!! Ridiculous! Show me proof of the phone calls!!!
