My documents for $600 rebate were returned as"undeliverable."

I purchased a $599 sewing machine with the written assurance from the retailer that it was eligible for a full purchase price rebate from United States Administrative Services, Inc., if I followed all the rules. I had email and USPS communication with the USAS claims administrator, Sandi Carillo, re: the proper documents.
After waiting 35 months, I followed the extensive instructions perfectly and submitted my documents for the rebate. My letter was returned as undeliverable, no forwarding address.
