Smarthchoice healthcare (not insurance). Bad enough it took me forever to process a refund. FINALLY got it ONLY because i disputed it with the credit card company.
TODAY i get a check and a letter in the mail from Assurity Life Ins.Co (lincoln, ne) asking if the enclosed copy of life ins. Application in my wife's name has her signature.
It wasn't even her SS no. They filled it out in texas saying she was an employee of NAPE - address correct, other info i had supplied.
Obviously NAPE is not averse to mail fraud (assuming the application was faxed/mailed to Nebraska).
I imagine Assurity may pursue this.
