I received a bottle of Colo-Klenz with no invoice by mail from Wellness Partners, 2122 S. 950 E., Provo, UT 84606-6163. I did not order this supplement, nor do I know how they got my credit card information. It was sent to my old address and forwarded to my new address. I checked my credit card statements and there it was, an unauthorized charge for $99.90.By dillard & yagen enterp la habra heig ca which were categorized as theatrical producers (except motion pictures) ticket agencies. I notified my credit card company of the fraudulent charge which they will credit back to my account.
