I visited the doctor office on November got an eye exam, fit and follow up for contacts, and 3 boxes of contact lenses. Back in December my company changed insurance company.in April I needed to refill my contacts and went back to Doctor Espinoza office requesting more contacts, they did an eye exam, and they wanted me to pay a $ 77.50 copayment for fit and follow up...
I did not agree to this as my prescription form November was still valid, they denied to give me the contacts if I wouldnt pay for the copayment, so I left the office and got my contacts somewhere else with my old prescription.
Doral Eye Center filled a claim to my new insurance company (Humana Vision) for the eye exam, fit and follow up and $138.00 worth of contacts lenses (which I never received from them)
I notified what happened to Humana Vision, 4 times I had to call customer service, the doctor office won't reimburse Humana the amount paid to them for the fit and follow up ($78.50) and won't give me the contacts or the new prescription.
I googled this provider and there are 4 other cases reported of fraud, they bill the insurance company for services not provided to their customers.
