Fund Administrators, LLC Promised to refund 50% of the cost of Lasik eye surgery, in the amount of $2,450.00. When claim sent it was returned, PLACE DOES NOT EXIST!

I am very upset!!! I was promised a refund of $2,450.00 for having Lasik eye surgery.
I sent the initial registration stub like I was instructed to do. (in 2007)
I then waited the 47 months to send the claim form for my refund. (June)
I received the envelope back, saying the place does not exist!!!
It cost me $20.00 to mail the info to them by registered mail! (twice)
I want my money!!! This is wrong, wrong, wrong!!!
They need to pay what was promised, after I followed their instructions did what I was asked.
