Sent in for rebate for my son but when checking status said invalid box top. Resubmitted and still have not seen a check

I sent in for the rebate for my son's contact lens purchase. My wife is an optician so I had her fill out all the paperwork. We receive a notice to check online for status and saw that our rebate was invalid because of invalid box top. This was a shocker as she sent in the end panel with the Rx (prescription) from each box as requested. Refiled a claim but still have not received our $60.
