I purchase a memory card and submitted the rebate on 6/27/07, following instructions to the letter. After 5 months waiting and numerous email requests, I was denied rebate. They claim that it is missing UPC code cut out. Since I made copy of everything before I submitted the rebate, I faxed them the copy which clear has UPC code. They cut off all communications with me: no emails, no call backs. This is a plain rip-off. I did a google on "US Modular Rebate Problem" and found many complains about rebate denial similar to my situation. Evidently, I am not alone on this issues.
