Rebate scam

I bought a memory from Cook's Technology in Santa Clara, CA in August.

Obviously I submitted the finished refund form, bill, and UPC.

I Have shipped in a large number of discounts before, I understand how to do that.

They sent me back a postcard stating "Sadly we couldn't recognize your receipt because of the following cause (s): $15 PNY Email in Refund - G-SDU16G2-FS/POL

Lacking UPC"

Today I've nothing, only a useless postcard, no type, no bill, no UPC, with no method to get OUR $15.: (
