Purchased 5Qt chef pan (because of rebate) from Bed Bath & Beyond. Mailed receipt, $10.00 rebate coupon, and proof of purchase within 15 days of purchase. (I have copies of all above mentioned) Purchased 12-18-06, mailed rebate 12-19-06. It is now May.
