This is the copy of the letter I sent to Social Security office on May 6:

To Whom It May Concern;

I am submitting this letter to issue a complaint regarding a matter of trying to cancel my participation with the Medicare Part B program. The matter has yet to be resolved and due to its difficulty, I decided to submit this letter to your office.

On March 12; I received a letter from Social Security stating that my monthly premium of $104.90 beginning in May. On March 25; I called the customer service number (1-800-772-1213) and spoke with Teresa. I expressed my desire to cancel my participation of the Medicare Part B; and was informed that she would input the request for me over the phone. On April 10 I received a letter from Social Security—once again—stating the amount of $104.90 would be deducted beginning in May. I contacted Social Security that same day&and spoke with Ms. Anderson. She informed me that I would have to submit a FORMAL LETTER indicating my request. Ms. Anderson gave me an address: Social Security Administration, Department of Disability Operations in Baltimore Maryland. I confirmed the address back to her. Then went to the post office and mailed the letter and paid $19.95 to send it EXPRESS OVERNIGHT DELIVERY. I received confirmation via email the correspondence had arrived the next day. I do have the proof of delivery.

Anyway, from April 10 through May 1; I have been in contact with the Social Security office via telephone. During each call, I was informed... By your telephone agents. That the matter was being "worked on" and should be resolved by my payment date on May 8.

On the last day of May, I had made several calls to the Social Security Office was now informed the original letter had not even been received in the payment center. On May 1; went to the Social Security Office at the Tip O'Neill Federal Building in Boston Massachusetts. I met with Ms. Ferring who was very kind enough to give me a form "REQUEST FOR TERMINATION OF PREMIUM HOSPITAL. AND/OR SUPPLEMENTARY MEDICAL INSURANCE" Which I filled out at the office. Ms. Ferring then imputed the information into the computer; and then issued me a copy the the form. This morning (May 6); I contacted the Social Security Center via telephone and was connected to Vince. He informed me—again—the request had not been processed. Later that day, I made a visit to the local Social Security office in Boston; and met with Ms. Valliere. She was most gracious to explain the matter to me which I was able to understand what was happening. Ms. Valliere... Was exceptional. She then informed me that my letter had been sent to the wrong office... Therefore, I was given an INCORRECT address to send the original letter to. I was given an INCORRECT ADDRESS by Ms. Anderson on April 10. I realized that had I made the visit to the local office in the beginning; the matter would have been quickly resolved. Because I was provided INCORRECT information by agents at the customer service telephone center; I am being inconvenienced.in part, I paid $19.95 for express delivery; only to have the correspondence sent to the wrong address. I would be happy to submit the appropriate documentations to you as proof. Because the the misinformation provided by Ms. Anderson in regards to the mailing address; I would like to have the amount of $19.95 refunded to me; in addition to the amount (s) of $104.90 premiums deducted from my check as soon as possible. I believe that I have done what was required of me, in regards to sending my request IN WRITING... Only to have been given INCORRECT INFORMATION from some of your agents... And now due to some mistakes made... I am being inconvenienced. So I decided to bring this matter to your attention in hopes of getting the matter quickly resolved.

This is the correspondence I sent to the Social Security office this afternoon. Due to nature of the circumstance; and my inability to get a resolution; I decided to bring the matter to your attention hoping to receive a prompt resolution.
