I completed an application form from simplefilings.com for a new social security card. I then submitted the form to the regional social security office as instructed on the form. The form was refused. The form is not acceptable for obtaining a new social security card. New social security cards are free and must be obtained from a social security office. I was told I had been scammed by the social security office personnel.
