Charged me $1500 for a ten year plan for body scans, now no telephone # is answered

I paid 1500 dollars for a ten year plan for body scans at heart check america, and now i have no contact with heart check america all telephone numbers are disconected idont no where to turn, i have the contract, provider id numbers telephone and fax numbers.
