Orlando Oral Implant Center—Dr. David Hazen
Filed claims with our auto insurance and was paid on work never done

On Sept. 29, my 28 year old son and myself, his mother, went to Dr. David Hazen for dental work. He took an ex-ray of my som, then took the busted tooth out of his mouth that had just been glued back in. He was in a car wreck and knocked one tooth out and chipped the post for the cap on the other front tooth. I was then ex-rayed because I had a wisdom tooth in need of pulling. Dr. Hazen had just put the x-rays up to look at them, and then my son suddenly fell forward like he was going to get up, and just rolled out of the chair head first. He had a huge knot on his head and he then went into a seizure. It was awfully scary! The assisantant called 911 and they took Robbie to the E.R. At FL. Hospital Altamonte Springs, FL.

On our way out of the office with the EMT's, heading for the hospital, the assistant handed me two cards which I threw in my purse since I was trying to keep up with the stretcher, and Dr. Hazen asked me if I wanted to take my sons old cap with me. I told him to just keep it until we knew what was going on with my son. Two days later I'm looking at my checking account online and I see a charge for $130.00 which was for my ex-ray, $35.00 and then $95.00 for a consultation, which we neither on got. They billed not only my Bank of america acct. But also my Washington Mutual acct. There was also a fee for $100.00. I went in the next day to reschedule, and while I was there, I asked if she knew anything about the $100.00 and she told me it was a "no show" charge. I hadn't even made any appointments, but she said she made them, without my knowledge, while the EMT's were working of my son.

The next thing I know is they kept adding charges, so I had to go in and cancell both debit cards to stop them from charging any more. I reported it to both banks, and they there all cleared off, or so I thought. I had gotten a letter from Bank of America stating it was credited to my account. Just today I was checking my Washington Mutual acct., and they charged me $130.00, paid to Dr. Hazen.

They had all the car insurance information so they could bill them directly for work done. Well they didn't do any work on us, but they showed on the insurance bill that they had done the work, and charge almost $8,000.00. That is pure and simple insurance fraud! I had also asked the assistant if I could please get copies of everything that we had signed, and any billings they may have had. She refused to give them to me. I said why. I was told I have a right to them. She told me to get out of their office or she would call security and have me removed. I did nothing or said nothing for her to react like that.

Two days later I took my son back for his appointment, and they had the door locked. About ten minutes later, Dr. Hazen comes out to waiting area and demands that I leave immediately, and that he had filed an injunction against me. I went down to the lobby to wait for my son, and when he did come down, he said Dr. Hazen refused to treat him, and referred him to another dentist. I later found my own dentist that got my son all fixed up properly, and politely. I had to pay cash front, but because Dr. Hazen had claimed all the money, I did not get my money nor did the Chiropracter, who we now owe $2,600.00. This is just not right.
