Mis-representation of CPT Codes in the Medical Bills

Hi,

There are two complaints:

(i) Mis-coding of the Health Services:
I had a CT Scan with Mills-Peninsula (San Mateo, CA) which of course needs a contrast material (Dye) to be injected in to the patient's body. My Insurance covers this 100% but any consumables reported along with this doesnt get paid by Insurance (goes against my deductibles).
There are separate CPT Codes available (74160 and 72193) which are the correct ones that needs to be used for CT Scan with Dye;
But for some strange reasons, they have reported this under 74170 and 72192 (which are actually the CPT Codes for CT Scan without dye) and have shown the Dye-cost as Hospital Incidentals.
I have spoken to the Business office of Mills Peninsula (Ms. Charlotte at 650-652-3030) but she is not ready to re-send the bills to my Insurance with revised codes. I have left her 2 voice mails so far and she doesnt return the calls also.

Complaint (ii):
They havent sent me any bills but have directly sent the account for collections. When enquired later, they specified that the bills were sent to a wrong address! This has started appearing in my credit report which is causing other problems.
