Charged for Pay Per View from 2003 when I cancelled the account in 2011

I cancelled my DirecTV account in December after having an account with them since and promptly received a bill showing a $25.17 credit. I then returned my three access cards as requested.in January I suddenly receive an email saying my account was past due by $56.94. I never received a bill for this amount and had no idea what this was about. I called customer service and they said it was for pay per view that I had ordered in 2003 and had never been billed for! I questioned how this could be and I was told that it was ordered through a receiver and as there was no phone line connected to my receiver I had not been billed for them. I questioned this as we never order pay per view because of the cost. I was told the information was on two of the three access cards and therefore someone in the household must have ordered the service. I asked if that meant that I could have ordered 1000s of dollars worth of pay per view through the years and I wouldn't have been billed for it until I turned in my access cards and I was told, yes, this was possible. How can this be? How can you run a business like this? I find this impossible to believe. Then they told me they would give me a credit of 34.95 and I would still owe 20.07. There is no way that I can dispute this I certainly do not remember what I was watching on TV in 2003 and I doubt if anyone else in the house remembers this either! I filed a report with the BBB and I want to receive my credit of 25.17!!
