My name is judy, i am 59 yrs old and disabled. I am on alot of prescribed meds. When i first noticed this on the bank statement, i called the bank, they found a phone number. I started my long journey. I have called about 25-50 times, finally i heard back from a young man, nathan. He was supposed to send a form for me to get 4 payment vouchers. Well i just received them this last week 10-17 thru 25th. Not sure memory lapses. I and my husband are living hand to mouth.
