Kohl's
After waiting 6 months, my $20 rebate has not come! - Philips Sonicare toothbrush, model HX6512/50

June 2 I bought a Philips Sonicare, model HX6512/50. Kohl's offered a $20 rebate for item purchased between 5/5 and 6/30 and if the rebate application was postmarked by 8/15. I met these requirements. I sent the original Kohl's rebate form, along with the original proof-of-purchase (UPC symbol) and my original Kohl's dated sales receipt. I checked status of my rebate with Kohl's on-line website and Kohl's says they have no knowledge of my rebate request. I kept copies of all that I originally sent.
