I DID return the equipment

I have TWC all year in NYC and every summer in Ellenville NY. I turned in the equipment in Ellenville in 10/12 as I do every year and turned off my service. When I called 6/29 to start up again I was told I owed hundreds of dollars. They had no record of the equipment being returned and had turned service back on in March in error. After TWO hours on the phone it seemed to be resolved and I was given an appointment for 7/5 to start new service. Today TWC called and said I had to pay in 3 days and they had added hundreds of dollars for the equipment and turned off the new service. Customer Care turned it over to Security (# 814871) but how do I prove I returned it? Actually my neighbor brought it in and I said we would both give them notarized statements but I don't know what is going to happen and I am NOT paying for equipment that was properly returned and service that I didn't get. I would like to get my new service back on.
