First of all, are you trying to DISCOURAGE reports? Then don't require PHYSICAL address. These scheming cheating companies won't give us physical addresses and YOU KNOW IT!
So, despite my suspicion of your intentions, here is my latest experience with one of the worst corporations for such rip-off schemes there is in this country -or the WORLD - AT&T!
I bought cellphones last month from Cingular - which was a decent company, but now is bought out by ATT - and sent in rebate materials and, of course, kept all copies and records.
Today, 7-14, I get mail refusing my requests because "[materials] were not submitted". This is a LIE. There is no question of what was in the envelopes (2 rebates).
So, I call the number listed on the letters and get a nice young lady (told her I hope ATT treats her better than her customers) and said since I know my submittals were complete and accurate this was another scam on the part of ATT and I intend to proceed with complaints including our state attorney general. Miraculously, after confirming just one number from my transaction records, she said the rebates were on their way.
