Incentive not received, all documentation sent

Dish Network lured us from Direct TV with several incentives. The free 512MB iPod Shuffle was never delivered. All the paperwork was sent in along with the money order for shipping and handling on November 13 with an authorization number from Evoke Incentives. As of 1-15 nothing is received.

I have called on 12-14 (voice mailbox is full try later), 12-28 (voice mailbox is full try later), 01-04-07 (left voicemail with phone number and email address), 1-9-07 (voice mailbox is full try later), 1-9-07 left message on Mr. Gomez's voicemail in option #3 which is actually sales but it was the only voicemail that would accept messages, 1-15-07 (voice mailbox is full try later).

Also sent emails to [email protected] on 12-14-06,12-28-06,1-4-07,1-09-07, and 1-15-07 with no response. I tried accessing their website today (1-15-07) and kept getting an error message; I would assume that means they no longer exsist.

I would hope Dish Network, who was the ones that offered this, would step up and take responsibility. I told their customer service people that I would not pay my monthly service bill until I received the incentive they offered. They laughed and said they would then turn off my service. Can anyone help on how to get answers on these people???
