The return address on the envelope is Wellness Partners, 2122 S. 950E., Provo Utah 84606-6163. The bottle has the phone number 877 455 5085 and the website www.24hourweightloss.com. After seeing other complaints for $99.90, I will be checking my accounts for a deduction.
