Terrible service, tried to ruin our credit

Where should I begin? Two main issues:
1) In in the process of running a credit check, we discovered they accused us of not returning a cable box in we owed them $140, and they had turned us over to a collection agency. Neither Time Warner nor the collection agency had made any effort to call us, bill us, etc. Even though we HAD returned the box, we would have paid the fee to prevent the expensive ding on our credit report. When we went to the local office and confronted them, they said they had no record, etc.in this case the company wins: WE DID RETURN BOX, THEY GOT PERCENTAGE OF DEBT FROM COLLECTION AGENCY, COLLECTION AGENCY GOT $140 FROM US TO PREVENT EXPENSIVE DING ON CREDIT REPORT (which would have cost us thousands.) Really, WHO KEEPS A RECEIPT FOR RETURNED CABLE BOX FOR 4 YEARS? (I think they know this)
2.) We moved to an apartment this month. Out of the blue, Time Warner called wanting to provide cable and internet service. I was very leary, but they implied to me they were the only service provider at this apartment. Sevice appointment was scheduled, for which I received two or three reminder calls during which I confirmed the appointment, then received two reminder texts. Of course the technician did not show up, after we waited for 5 hours at home. When I called dispatch, they told me there was no record of the appointment.
