This is a very deceptive order form. Writing on it that it is Federal Labor Law Notice, as if they are associated with the federal govt. As if we do not comply & order these forms we will be penalized & fined.
These reports are free from the govt. But several people still think they have to do this because of this false, deceitful order forms. This is against the law & false reporting. These people should not be allowed to do business & should be fined a large amount for their deceitful order forms.
