CreditGift.com. - Chase Bank ripoff - Did not ship free DVD - No response from e-mail request

I submitted a application through CreditGift for a VISA card from Chase, in response to offer for a free DVD player.

Received my credit card, made my first charge, submitted 2nd VISA invoice and DVD Certificate to CreditGift. DVD has not been shipped and they will not respond by e-mail and I am unable to find a telephone number for them.

Similiar problem submitted on 10/03.
