I visited your restaurant at the Polaris Mall in Columbus, Ohio today, 5/15/10, and I purchased some cheesecake and breads. The clerk told me to go online to your feedback site and do the survey and I would receive a $10.00 gift card. My survey code number is: 4104-50041-14051. I have searched your site and cannot find any place to complete a survey. Why would you have that printed on receipts and then give customers the run-around when they try to comply? Shame on you.
