I have been informed by mail that I was awarded an amount of $10,000.00 US= Lump Sum Payment. Cash Payment Recipient; Gilles Coulombe I.D. 9034694910
Payment of Award is confirmed to the Named Recipient with my address On Record
Below the Payment Document, it was added:

Be it known and authorized by our offices of greenfield associates that the person named and identified upon this document is the known recipient of cash payment by cheque from international cash award. This document represents a real and actual cash award payment.

Attached there was a Payment Claim Document to be filled. A request of payment of 34.71$ Can to Greenfield Associates for Recipient Level payable by cash, cheque, MONEY ORDER or by credit card.
