It tells you the amount, and that you are identified.
Half way on the document it states again how much I will received then asked for $19.99 to be sent by cash, check, or money order payable to DRD Main Office - Payment Director Receipt of payment enacts Delivery - Stamp Bottom Sealed for filing and asked for my signature
