Directv cancels my account without notice and charges me cancellation fee

1. I had Directv, paying by automatic credit card payment. I changed my account number notifying Directv, and other companies. The only company not changing it was DirecTV. The charge did not go through and they cancelled my contract without notice. I immediately paid the bill as soon as I was aware, but they did not restart service. They charged me an early termination fee, equipment non return fee, and service downgrade fee. They had my address, email, and phone number but made no effort to contact me. They said they tried to bill me, but gave an address in another state which was never my mailing address. I requested they either give me credit for the termination fee in service or a refund. I offered to return their receiver. They said they would look into the situation, but 3 months later have done nothing.

2. A more minor issue, but when I started service over a year ago I confirmed the exact amount of my monthly fee, and the rep repeated it back again. I got my bill and it was for $5 more. I called and they said there was an additional fee for (1st) receiver rental. I asked if I could use an older receiver I owned and avoid the fee. They said I could use my own receiver, but I'd might as well use theirs, as they still had to charge me a receiver rental fee whether or not I used their receiver or mine. This was not what I was told when I signed up.
