Did not pay for services rendered to doctors or hospital

This health insurance company required pre authorization to attend doctors or proceedures. They did not pay the doctors or hospital for services rendered. Now we are getting bills from the providers seeking payment. We can not reach Beech Street and are totally being given the run around.
