Negligence - store negligence

I was pushed through the revolving door at CVS at 400 W. Madison in Chicago yesterday t approx. 3:15pm. I came out the end and was thrown on my back.
No personnel offered help to me. Two customers helped me up.
I asked for the manager (I later found out he was the asst. Mgr.). I asked why the handicapped door only worked from inside (I am disabled). He claimed the vendors lock them.
I went to the store this morning to file an incident report (which should've been offered to me yesterday).
Thank God I am not seriously hurt. As soon as I was stood up my left shoulder, ribs and foot hurt me. I found blood on one of my left toes when I got home.
CVS was extremely negligent yesterday. The asst. Mgr who tried to file an incident report this morning was not able to because she said there were only employee forms - there wern't any for customers.
If it is true that vendors are allowed to lock store doors, that would be outrageous, if not illegal.
This shows how little importance CVS places on its customers.
I was hurt, I was treated callously and I was not given a chance to file an incident report.
What does CVS have to say for itself?
