CVS complaints: where to take a pharmacy, Caremark or privacy problem

This page was checked on 2026-10-20. cvs.com showed our automated browser a notice that it is not available outside the United States, so we could not read its customer-service, pharmacy-policy, refund or transfer pages and do not quote them here. Use the pharmacy or store manager first, then the channels below, and keep prescription labels, receipts, order numbers and the dates and names of people you spoke to.

  • CVS customer service. The notice on cvs.com says its customer service team can be reached at 1-800-SHOP CVS (1-800-746-7287), and the cvshealth.com Contact page lists the same number under corporate headquarters, 1 CVS Drive, Woonsocket, Rhode Island 02895. cvshealth.com says that for detailed support you should use the brand site (CVS Pharmacy, MinuteClinic, CVS.com and app, or CVS Caremark).
  • CVS Caremark (prescriptions through a health plan or employer). The Caremark contact page says to call the Customer Care number on your member ID card; it also offers chat (not yet available to all customers, per the page) and a secure message center. The page shows no phone number or complaint procedure. Its footer links to a "Member rights and responsibilities" page that we did not read, so we do not state what it says about grievances or appeals.
  • Website and app privacy settlement. A class settlement over alleged sharing of CVS website and app user data is open: the official settlement site (read 2026-10-20) says users who accessed CVS digital properties in the United States before July 27, 2026 are class members and that a claim filed by November 16, 2026 is the only way to receive a cash payment of up to $5.00 without documentation or up to $10.00 with proof of membership in the Settlement Class. The same site lists an opt-out deadline and an objection deadline of November 1, 2026 and a final approval hearing on December 1, 2026 at 9:30 a.m. EST by Zoom, and gives the settlement administrator's phone number as (888) 654-1271. Payments follow only if the court approves the settlement and after any appeals. See the litigation section for the terms and the defendants' denial.
  • Pharmacy-level problems. State boards of pharmacy act on complaints about pharmacies in their states; BBB's profile, for example, reports an Ohio Board of Pharmacy fine against one Canton store (see below). Your state attorney general also takes consumer complaints.
  • BBB. BBB accepts complaints through its profile for CVS Pharmacy, Inc.; the figures are in the complaint-record section.

The federal CFPB consumer-complaint database handles financial products; a company-name lookup for "CVS" and for "Caremark" returned no companies on 2026-10-20, so pharmacy and benefit problems are not tracked there.

Government and regulator matters

U.S. Justice Department, controlled-substance dispensing ($315 million). A Justice Department release dated October 13, 2026 (No. 26-1170) says CVS agreed to pay $315 million to resolve a case in the U.S. District Court for the District of Rhode Island, United States ex rel. Estright v. CVS Health Corporation, et al., No. 22-cv-00222, which the government joined with a complaint filed December 13, 2024. The release covers October 17, 2013 through December 2, 2025, says the payment is made by CVS Pharmacy Inc. and related corporate entities, and states: "The claims resolved by the settlement are allegations only and there has been no determination of liability." It also describes a separate memorandum of agreement with the DEA that requires CVS to set up a hotline for reporting suspected illegal dispensing, proactively monitor dispensing at its pharmacies and set up a process to evaluate prescribers suspected of illegal prescribing. A news report (Claims Journal, Oct. 14, 2026) quotes CVS as saying it disagreed with the allegations and was not admitting wrongdoing. The whistleblower's share had not been determined.

Federal Trade Commission, insulin and pharmacy-benefit practices. In September 2024 the FTC filed an administrative complaint against the three largest pharmacy benefit managers, including CVS subsidiaries, alleging, in CVS Health's words, anti-competitive and unfair practices that artificially increased insulin costs. On July 14, 2026 the FTC announced a proposed consent agreement with Caremark Rx, LLC and Zinc Health Services, LLC, accepted for public comment by a 1-0-1 vote (30-day comment period). The FTC release lists proposed requirements including a standard option that passes rebates through to members at the point of sale, insulin out-of-pocket affordability programs, and a monitor for complaints about interference with pharmacies' use of hub service providers; the FTC says the settlement could lock in up to $8.5 billion in consumer savings over 10 years. CVS Health's 10-Q says the proposal is subject to public comment before the FTC decides whether to issue a final order, and that the FTC has halted its litigation meanwhile. We did not find a final order in the sources checked on 2026-10-20. The FTC release does not state whether Caremark admitted wrongdoing.

Louisiana Attorney General. Local TV (KPLC, Feb. 20, 2026) reported a $45 million settlement of three Louisiana lawsuits against CVS Health and its pharmacy benefit subsidiary; the report says the suits alleged that CVS misled consumers about pending legislation, broke regulatory rules in its pharmacy-benefit work and engaged in unfair competition affecting independent pharmacies, and that the settlement was reached without CVS conceding liability. We did not open the attorney general's own release.

Insulin pens, False Claims Act (U.S. Attorney, S.D.N.Y., $37.76 million). A U.S. Attorney's Office release dated December 2, 2025 (No. 25-250) says CVS Pharmacy, Inc. agreed to pay a total of $37.76 million, of which $24,446,240 goes to the United States and the remainder to various states, to resolve a suit (No. 18cv3047) over billing and dispensing of insulin pens to patients in government healthcare programs from 2010 through 2020. The government alleged premature refills, more insulin pens dispensed than prescriptions called for, and under-reported days-of-supply. The release states that CVS "admitted and accepted responsibility for certain conduct", including that "CVS pharmacies dispensed more insulin to GHP beneficiaries than they needed". BBB's profile separately reports a July 9, 2026 settlement with the Massachusetts Attorney General, the Justice Department and 35 other state attorneys general over the same insulin-pen allegations, with CVS agreeing to pay $36.5 million to the federal government and participating states. Those two figures come from different sources and are not added together here.

Vermont Attorney General (pending, as reported by BBB). BBB's Government Actions alert says that on July 17, 2024 the State of Vermont filed a complaint in Vermont Superior Court, Washington Unit, against CVS Health Corporation, CVS Caremark-related entities, Express Scripts-related entities and other defendants, alleging violations of the Vermont Consumer Protection Act by pharmacy benefit managers. BBB labels this a pending action and a summary of allegations that have not been proven. We did not open the complaint.

Ohio Board of Pharmacy (as reported by BBB). BBB shows three government-action alerts on this profile (Ohio, Massachusetts and Vermont, as displayed on 2026-10-20). The Ohio alert says that on February 6, 2024 the Ohio Board of Pharmacy fined CVS Pharmacy store #2063 in Canton $250,000 after an investigation begun in 2021 that, per BBB, found delays in obtaining prescriptions, phones not working properly, lack of appropriate drug security and control, and failure to provide a safe working environment for pharmacy staff. We did not open the Board's order.

National opioid settlements. CVS Health's 10-Q for the quarter ended June 30, 2026 says forty-five states, the District of Columbia and all eligible U.S. territories are participating in a settlement resolving substantially all opioid claims against Company entities by participating states and political subdivisions (not private plaintiffs), and that the company separately settled with Florida, West Virginia, New Mexico and Nevada. The settlement agreement is at nationalopioidsettlement.com. The same 10-Q states that as of June 30, 2026 the remaining accrual for these opioid matters was approximately $3.4 billion, that the State of Maryland has elected not to participate (litigation is pending with certain Maryland subdivisions and other non-participating subdivisions), and that in May 2026 a Florida state court entered a directed verdict in favor of the company and other defendants in a case brought by a group of Florida hospitals, which the plaintiffs have noticed an appeal.

Who operates CVS: CVS Health, CVS Pharmacy, Caremark, MinuteClinic and Aetna

CVS Health Corporation is the public company that files with the SEC (CIK 64803) and is named as a defendant in the federal cases below. Its corporate headquarters is 1 CVS Drive, Woonsocket, Rhode Island 02895 (cvshealth.com Contact page). SEC filings for fiscal 2011 to 2013 carry the name CVS Caremark Corp and later filings CVS Health Corp under the same CIK.

The cvshealth.com About page (read 2026-10-20) lists the brands Aetna, CVS Caremark and CVS Pharmacy and says the company has "more than 300,000 colleagues" and "more than 100 million people" engaged; these are company statements. BBB's profile is for CVS Pharmacy, Inc., the retail pharmacy business, a corporation at 1 CVS Dr, Woonsocket, RI, which BBB says offers health and beauty products, other retail items and prescription fulfilment, and BBB lists MinuteClinic, L.L.C., CVS Caremark and Longs Drugs as related businesses. CVS Caremark is the pharmacy benefit manager: it works for health plans and employers, so a Caremark problem (mail-order delivery, plan coverage, prior authorization) is a different relationship from a problem at a CVS store counter. In the court filings quoted here, pharmacy-benefit defendants include CaremarkPCS Health, LLC and Caremark Rx, LLC.

Omnicare, a long-term-care pharmacy, was a CVS Health subsidiary; the 10-Q says Omnicare began a Chapter 11 case on September 22, 2025 and was deconsolidated that quarter. Aetna is a health insurer and is a separate company profile on this site, not covered here.

Complaint record: BBB, Trustpilot and this site's archive

BBB (profile for CVS Pharmacy, Inc., Woonsocket, RI, covering headquarters and corporate-owned locations), as displayed on 2026-10-20: not BBB accredited; BBB Rating A+; "2,063 complaints in the last 3 years" and "586 complaints in the last 12 months"; customer reviews "1.09 /5 stars, Average of 410 Customer Reviews". BBB states that customer reviews are not used in its letter-grade rating, and that the nature of complaints and a company's responses are often more important than the number given the company's size and volume of transactions.

Trustpilot, as displayed on 2026-10-20: 1.5 from 3,128 reviews for CVS Pharmacy (www.cvs.com); unclaimed profile; Trustpilot states it does not fact-check reviews. These platform figures are shown separately and are not added to each other or to this site's count.

This site's archive. 650 approved complaints dated 2000 to 2015 name CVS, CVS Pharmacy or CVS Caremark as the subject; five more are linked only as related (a store parking-lot booting, a prepaid card bought at a CVS store, a towing complaint, a mail-order service that said it would become Caremark, and a Target-store MinuteClinic visit). Complaints with other businesses that merely use the letters CVS (for example a timeshare telemarketer) are not counted. The posts are individual, unverified consumer accounts, all at least ten years old, and they cover many separate stores. By keyword match (a post can match more than one theme): about 241 concern mail-order, Caremark, coverage or delivery; 283 mention prescription errors, refills or pharmacists; 167 mention pricing, coupons, ExtraCare or refunds; 351 mention staff or manager conduct.

Court cases and settlements

The descriptions below come from CVS Health's Form 10-Q for the quarter ended June 30, 2026, and from the Justice Department, unless stated. Allegations are not findings.

Omnicare False Claims Act case (U.S. ex rel. Bassan v. Omnicare, Inc. and CVS Health Corp., S.D.N.Y.). The U.S. Attorney's office intervened in December 2019, alleging that Omnicare filled prescriptions for certain non-skilled nursing facility residents without valid prescriptions. In April 2025 the jury found Omnicare and CVS Health Corporation liable and awarded about $136 million due to Omnicare's conduct (trebled by statute to about $407 million), with no damages attributable to CVS Health Corporation. In July 2025 the court awarded penalties of $542 million against Omnicare and found CVS Health Corporation jointly and severally liable for $165 million of that. Omnicare filed Chapter 11 on September 22, 2025. In July 2026 CVS Health Corporation entered agreements with the DOJ and the creditors' committee, subject to Bankruptcy Court approval and an asset sale, under which the DOJ receives a minimum of $440 million from Omnicare and CVS Health, CVS pays $130 million within two weeks of the effective date, and the appeals are then dismissed. Healthcare Dive reported on 2026-09-21 that Judge Stacey G. C. Jernigan of the U.S. Bankruptcy Court for the Northern District of Texas approved Omnicare's wind-down plan (a trade-press report; we did not read the order and do not state that the appeals have been dismissed). The 10-Q also says that if the DOJ does not receive at least $310 million in payments from Omnicare by March 15, 2028, CVS Health Corporation will guarantee the shortfall.

PBM Medicare Part D reporting case (U.S. ex rel. Behnke v. CVS Caremark Corporation, E.D. Pa.). The government declined to intervene. After a two-week trial the court ruled CVS liable under the False Claims Act as to certain claims in a split decision and entered a judgment of $291 million after trebling and penalties; CVS Health has appealed to the Third Circuit. We could not complete a docket check (CourtListener showed a security check), so the docket number and the district-court docket status are not stated here.

Controlled-substance dispensing case. Settled for $315 million as described in the regulator section, with no determination of liability.

Website and app data class action (Brewer et al. v. CVS Pharmacy, Inc. and Criteo Corp., Broward County, Florida, No. CACE-26-008094). The official settlement site says the suit concerns alleged disclosure of user data of CVS digital properties to Criteo or other technology providers, that "Defendants deny that they violated any law", and that the settlement does not determine which party is correct. It offers cash payments of "up to Five U.S. Dollars ($5.00) without documentation" or "up to Ten U.S. Dollars ($10.00) with proof of membership in the Settlement Class" to class members who file a claim by November 16, 2026. The settlement site (home page, read 2026-10-20) lists November 1, 2026 as the deadline to exclude yourself or object, November 16, 2026 as the claim deadline and a final approval hearing on December 1, 2026 at 9:30 a.m. EST by Zoom, and says the court still has to decide whether to approve the settlement. The home page does not state a total fund amount. The settlement is not final until the court approves it.

The 10-Q also describes other pending matters, including a securities class action over alleged omissions and misrepresentations relating to the profitability of the Health Care Benefits segment in the Southern District of New York (a motion to dismiss was filed in May 2025) multi-district drug-pricing litigation in New Jersey, and Roofers Union Welfare Trust Fund v. CaremarkPCS Health, LLC, et al. (N.D. Ill.), in which a putative class of Caremark clients filed a complaint in March 2026 alleging breach of contract, unjust enrichment and RICO violations over payments Zinc Health Services received from manufacturers (allegations only). We summarise none of them here beyond noting they exist; the filing is the source.

Is CVS Health Legitimate?

CVS Health Corporation is an SEC-reporting company headquartered at 1 CVS Drive, Woonsocket, Rhode Island, and BBB's profile for CVS Pharmacy, Inc. gives a business start of 1963 in its page data, a BBB rating of A+ and no BBB accreditation (as displayed on 2026-10-20). The same profile shows 2,063 complaints in three years and 410 customer reviews averaging 1.09 of 5, and Trustpilot displays 1.5 from 3,128 reviews, so ratings from different sources differ widely and are not combined.

Government matters are documented above: a $315 million Justice Department settlement announced October 13, 2026 with no determination of liability, a proposed FTC consent agreement subject to public comment, and False Claims Act judgments in the Omnicare and Behnke cases that CVS has appealed or agreed to resolve. Those are the primary-source facts; they say nothing about an individual store or order. Complaints about a particular pharmacy are best reported to that store's manager, CVS customer service and your state board of pharmacy.
