Harrasment at the work place verbal abuse. Numerous times at during morning meetings and on the telephone. Mrs peterson is in fiance and i am a nurse in clinical. Ms. Peterson will call me upset when i have to send a resident to the hospial on one incident she want to know why i sent resident to hospial because she has medicare and i should have made a clinic appointment. This resident was diagonse with suffering fron stroke. I was informed that if i sent resident to hospial the nursing home would loose money and that i should have the doctor treat the resident within the nursing home. On another ocassion i rec'd a verbal warning because i refuse to allow a resident to be discharge from facilty on her order againist medical advice. The doctor instructed me that the only that can say a resident can go home against medical advice is he not the fiance director i was given a verbal warning for giving the md the proper discharge paper at his request. On another ocassion ms peterson scream at me because she wanted me to give her the my plan of care on another resident. I told he he has chronic apiration and as an lpn i need to keep resident from further apiration she the insist i tell her my plan of care i the told her if she needs more infomation she has to speak to his doctor. Later i went to my dns and ask her why she sat there and allow her to harass me because she knew as an lpn i am un able to do a place of care i am not allow to acess or stat a care plan dns stated the in the pass told them that resident alway use to spit. On another ocassion i was harrass by both she and the adminstior because i could not get the doctor to sign off on a md order that the rn they just hired ask a lpn to put a doctors order into the system without informing the md. I am alway told to move residents around to acommade new resident. I was told ms peterson that she do not care if the resident do not want to move it's not thier decion it's hers.in some cases resident family was not to about the move and in other cases the lied to family and the resident by telling them they and emergency. On 2/5/07 the facilty was being servy by the department of heath and in perparation for servy. I ask both doctors if i can order all ppd and lab and all other changes so we would not rec. D and deficency dr. Umeror alway give me the o. K to fix all pharmacy deficency and i ask dr hussin one week before sevey because i placed lab results and request for ppd that was pass due in his commication book and have been sitting their for two week. I saw dr. Husain in the elevator and i him is it o.K. To order ppd lab and any changes needed because i'll be working on those over the weekend i not sure if he heard everything i said his respond was ms callender you know you know you do while the elevator doors continue to close because md is alway in a hurry. Durin the state survey the survey not that the nurse was given medication at the wrong time. She told me that our three way system is not work because out of three nurses someone should have noted the error. I told he that we was sorry she than said we need to fix it because survors will be back tomorrow it hen went into the system and toor out one of the frequency so the the next mouth the nurse picking up the md order would be force to write in he time as print on the md order in stead of the system the printing wrong time for diabetic medications. These error alway showed up on pharmacy deficency and the doctor always asked me to put in the correct time. I did this on 2/05/09 after the survoy left the floor. I was called that evening by my (dns) director of nursing that i am not to come to work because they are doing and investigation. On 2/6/09 i was told by my dns that i made changes on a doctors order without the md knowledge they called both doctor and doctor umezor was that instucted me that i can make changes on medicaton times base on pharmacy recomendation. Dr. Husain only stated that he did not speak to me that evening but in the pass dr. Husain alway also instructed his nuses to put in the correct time on pharmacy deficency. Dr. Husain gives all the nurses his pass word to do his monthly doctors oders. We have to read his progress notes to order medication lab and test and any other orders needed for each of his residents under his pass word. Doctor husain never wanted to do his own order like dr. Umerzor and both md was trained to do so the system has been in effect for two years. Supervisor was made ware of this behavior and dns has knowledge of his way of doing things one supervisor refuses to use his pass word. On 2/5/08 i did not change any medication ordered. I did not change the dosage i ony took off one of the freguency that keeps giving the faciliy deficieny caught by pharmacy and now the surveyor. And for this i've been suspened indefeanity. For changing a time that was causing the nurses to give the medication at the wrong time. But 5 months ago they but tired to protect the nurse by using me to have the doctor to sign off on a medication that he did not authorize. Dr. Umeozor was very angry when gave him the md order md told me that i should give the order to the dns and that she can to with it what ever she wants. That afternoon i was to by ms peterson to come to the coference room now i tod ms peterson that i was in the middle od adminstating medication and that i was the only nurse on the floor and that i could not come now. Ms peterson sceamed at me and said that the adminastrator wants to see me now. At that time the supervisor came to the floor. I had to tell aa the resident wait for their medications that i'll be back. When i got to the conference room the adminstator mr. Orrila fiance director mrs peterson dns mrs remsemmair and the rn who instructed the lpn to put the order into the system without doctors permission mrs germain. Once i sat down the adminstrator started yelling at me. He said ms callender i thought you said that you was going the fix the admission and all the order why didn't fix it. I told the adminstrator i did go over the admisson with the md he signed off on every except that md order be cause he did not give the nurse the order to do so. The adminator start screaming no one is going to loose their license today. He than asked me and ms germain to leave the room. Around 2pm t rec'd a telephone form mrs peterson which was confrence in by the rep fom IS the computor advisor as per ms peterson asking me was their anything else needed to be remove at first i didn't know what she was talking about until i heard the rep from is voice. Ms peterson had the order remove in case someone was to do an investigation. I was told that orders can not be removed from the system but you can place them into another file when i later asked another computer rep. After i went into the computer to see if the dr's order was removed and they was. Someone called the state in reference to the order at the time of the investigation i was out sick. I was to that adminstation lied to the state when she ask to see the md order they said there was no such order. Three month later the investorgator came back this time i was present. I went to the union rep and told her i've was told to come to my dns off and speak with the investigator from doh the uion rep stated to me that i have my owm intigurity to thinh about so went to see her and told her truth about the md order the in vestigator told me that now that i told her the truth and that perviosly every denied that she wanted to see the md order. I asked the investigator if she rec'd the md or would any of the staff lose their license she looked at me for a moment and said no i just want to see the md order. When i left the office i told the dns that the investigaton will not cause anyone to lose their license. That she just wanted to see the order. After the meeting a copy of md order was given to the investigator and action was taken against the nurse by the investigatior as she told me she wouldn't.
