Received the same check w/mostly all the same information... Except the phone number was 1-416-830-5446, it was in conjunction w/Safeway, and sponsored by Wal-Mart, K-mart, HSBC, Costco, Sears and many many more major Stores... Thank goodness for this sight, I was not going to call until I found out more information.
