Received a prize claim notification in an envelope with a return address to CostCo Wholesale. The claim stated that I won a mega jackpot on 12/25 and my winning #s were 40-56-78-81-03. It states that after many unsuccessful attempts at contacting me I am only entitled to $150k instead of $3M. A check for $3990.00 was enclosed, drawn on Cathay Bank, in Los Angeles, CA. It also states that $1995 of this check has to be paid to the tax agent but I have to call for instructions first. The contact name is Lloyd Lewis and the # is 905-598-5410. This is a scam... Please do not fall victim to this as your bank account will be harmed.
