Chg for TV Receiver # 2

I called and requester the RCVR #2 be removed from my package. Now six years later the receiver is still in my package and I am being charged for it. I have removed the antenna for the RCVR in late 2006.

I switched to HD TV and requested the rcvr #2 be deactivated. TheAccess Card: 4 Card # 0005 6534 3308
