I too was ripped off by askberkeley.com but I did find a way to cancel the "managed care" that I never signed up for. I want to list it here for anyone who needs it.
I went to "www.askenzyte.com" and was taken right to the Berkeley site. I then clicked on altovis on the left of the page and found the cancellation form. I hope this helps anyone wishing to cancel something they never signed up for.
