Arlene Rose Gayares R.N. License # RN47250 Has been the acting Director of Nursing and QA for VIP Home Health Inc. For the past year or so. VIP was audited last year requesting 40 charts, and subsequently a total of 120 charts due to inconsistencies with nurses notes, nurses signatures, and billing issues. Arlene has been altering nurses notes and dates in order to complete those audited charts. She continues to ask nurses to change notes and dates on visits done over 6 months. We all know thats illegal. We have made complaints to the Board of Nursing in order to maintain our integrity and reputation in the Las Vegas community. Arlene has had disciplinary actions by the Board of nursing in the past, and that is why she continues to bounce from one agency to another. We refuse to alter notes per Arlene's request. This is illegal!!!
