This all it pays not a cent more... The maximum it pays for a surgery is listed there... And then its a schedule. So if the maximum it pays for a surgery is 3,000... But say for instance the schedule for a gall bladder is only 1,500.. This is what you will be paid... The 1,500 and if the surgery is about 10k then you will owe the rest
