Unauthorized charges to bank account ripoff

I have received monthly bank statements with $20.95 deducted from my account. I never agreed to this from any company. My statement has the bill credited to Lsm5-us-pay.com. When I attempted to contact them through to internet it would only reply error on page. When I attempted to call the phone number provided 866-221-5391 I only received a recording and then it automatically hung up on me.

This company is embezzling funds from my account and I want it to stop immediately!
