Ripped off for $150 rebates, first missing bill, then computer lost all information, then company moved and lost my rebate form. Said they mail me stuff, never got here, now they say it to late

Ripped off for a $150 customerloyalty rebates on a cellphone ordered on line, phone service was with Verizon. I have dealing with this for 10 months.

First I was missing one bill I know was theree when it was sent in, so I faxed it to them everything was there. Then their computer lost all information, had to re-enter it. Then company moved and lost my rebate form, when it was there before. Said they mail me stuff, it never got here. Someone was to call me, no one ever called now they say it to late I have lost the $150 rebate.
