Failure to honor cashback promotion

I purchased a Viking sewing machine for $2799 from Country Stitches in Flint, Mi based on the promotion that after three years the purchase price would be returned if all the proper paperwork was completed. The paperwork was sent 10/30/07 and after 16 weeks there has not been a reply.

The phone number listed has been disconnected and web site given will not enable an answer.
