I looked in my bank account and a company called Gallaxy Benefits had taken out 19.95 for no reason. I want answers now

I looked at my bank account and noticed that a company called Galaxy Benefits had taken out 19.95, which I have no clue what for. There was a phone number to call and when I called it it was some disconnected phone line. The company has done this to me more than once. I have no idea how they got my information or what they are charging me for.
