I initally purchased this injury, hospitalization and disability policy on April 17,1972 and have never missed or been late with a premium payment. This policy was issued in my name and my name alone. I have never authorized any changes to it in any manner what so ever.
On 14 Nov at 6:30 pm in broke my left ankle while patroling a job site. I was taken to the hospital and had surgery the next day, to repair the ankle. I now have two metal plates and around 15 screws in the ankle. I will not be able to return to work until late April or early May. I followed the required procedures and submitted the required forms and medical reports.
I get a letter of denial of benifits as the insurer claims the policy belongs to a Nancy Ellesson and not me! Called them with policy in hand, gave them dates and all the info on the original policy. They were still adamant that the policy was not mine, based on their computer records. I have mailed them copies of the original policy several times since the first letter of denial. They still have not honored their commitment. I filed a complaint with the Hawaii insurance commision which is still pending. This borders on identity theft and outright fraud.
