Won't Send Monthly Statement and Harasses Me With Calls

I was in bankruptcy court three months ago and got out because they were the only company I owe.

They call me continually asking me when I am going to pay the back payment and as I told them when I got behind I had just had cervical procedure (repairing my broken neck) and I have only medicare and had many hospital bills to pay; and still do however they don't seem to understand the gravity of my situation and when I am home and in much pain going through physical therapy I don't feel like talking to anyone;

Now I have always sent them the monthly payment.
The harassment is just a little too much for me to handle; but now my main concern is that they won't send me a monthly statement and I send it with a letter attached for the past three months; yet no statement.
