Ripoff Inaccurate Reporting, Damaging Credit Cant refinance because of them

In June My bankruptcy chapter 7 was discharged, all of my debts were wiped clean. I worked very hard to rebuild credit for two years afterward.

I fought Transunion for improperly reporting on every item that was discharged, including (but not limited too) First Area Federal Credit Union.

I bought my house in June of the bank that gave me the mortgage pulled all three bureaus and apparently nothing was showing up (except my bankruptcy.)
I attempted to refinance the house in 2005 because my payments were high, but I was denied because First Area Federal Credit union had a lien against my house. (on a discharged debt?)

I went to the credit union and told them there had to be a mistake, and they apparently went to the court house and listed the debt as discharged, and that I owed nothing.

I then sent a letter off to all three credit bureaus showing that the debt was indeed discharged.
I never gave it another thought, thinking the problem was taken care of. (my mistake).

I decided to give it more time before refinancing, thinking with this problem taken care of, and being able to pay off some other smaller debts, my score would come up, and I would be given better rates, and monthly payments when I finally did a refi.

I attempted to refinance again today, I was dealing with Penn West Home Equity, and she informed me that there is a judgement against my house from (guess who...) First Area Federal Credit Union.

I asked her where this info was coming from seeing I had a court paper showing the debt was discharged, and she informed me Transunion was reporting this as a unsatisfied debt.

What do I need to do (that i havent tried yet) to get this off of my report, it is pulling my credit score down, and I being charged higher interest rates, and I am unable to refi untill this is taken care of.
The lady at pennwest even said after four years (since being discharged) this should not be showing up on my report anywhere..

If anyone can help me, I would be forever grateful.
