Credit card ripoff unauthorized debiting of checking account

I noticed today that there was a charge to my checking account for $19.95 by USA Benefits. I never authorized this or signed up for anything like this. I don't know how they got my banking information. My bank is good about this... They will credit my account and go after the company for reimbursement.
