Comcast Xfinity - adding fees when calling - xfinity cable and internet

I've been with Comcast Xfinity for 2 years. Their Internet service is fast and has only been out a handful of times since service.
Today I just happened to notice an extra xfinity tv fee - about 17.95/mo plus 1.28 fee for a converter box on my bill with a 7.95 discount. 1.28 month
I do not look at all my bills unfortunately. Especially the autopay bills - I have this on my Amex card. I had called last year for a service issue - it took two trips to fix. But what I didn't notice was this xfinity tv fee added plus the converter box.
I called in today to have it removed and the fees refunded. They were able to remove the fees but were only willing to refund 2 months. I never received the converter box nor did I ever order tv - the wife and I are saving money by using an antenna for local tv.
I was on the phone for about an hour - the rep first said her supervisor was not present then she said she could talk to another super and so on - not willing to budge on this.
I would like my money refunded - I am no longer using autopay - and would like verification by phone for all added services.
Shame on you xfinity!
